Ryuko was born the daughter of Soichiro and Ragyo Kiryuin, a pair of scientists who sought to unravel the secrets of the alien Life Fibers: clothing-based life forms with the ability to grant humans tremendous power. Shortly after her birth, Ryuko was the subject of an experiment to implant Life Fibers directly in her body. For unknown reasons, Soichiro drew the line at exposing his second daughter to Ragyo's cruelty and faked Ryuko's death during the experiment, taking care of her in secret until he was fully prepared to escape Ragyo and take Ryuko with him.
Soichiro changed his name to "Isshin Matoi" and dedicated his life to thwarting Ragyo's schemes. Isshin founded the Nudist Beach organization to oppose Ragyo, created the Scissor Blades to destroy the final evolution of Life Fibers—COVERS—and sewed a garment out of Life Fibers augmented with his daughter's DNA to bring out Ryuko's true potential. Meanwhile, Ryuko grew up alone, shuffling from boarding school to boarding school and learning how to fend for herself.
When Ryuko was seventeen, Isshin called her back to the manor to inform her of the war against the Life Fibers, but he was found out by Ragyo and assassinated by another one of her child experiments, Nui Harime. Ryuko arrived to find one of the Scissor Blades stuck in her father's chest. After pulling the weapon out of his chest giving it to Ryuko, Isshin implored her to stay so he could explain the full scale of Ragyo's schemes with what little time he had left, but Ryuko elected to chase after the mysterious killer instead. This saved her life, as the manor exploded shortly after she left it.
Six months later, the search for Isshin's killer led Ryuko to Honnouji Academy, a titanic school with an entire city built around it to support its rigidly-enforced hierarchy. There, she made friends with Mako Mankanshoku and found a prime suspect in Satsuki Kiryuin (who, unbeknownst to either of them, was her older sister), tyrannical leader of the student council.
Before Ryuko could challenge Satsuki to find out the truth, she was beaten by a bearer of a Goku uniform, clothing that was partially made out of Life Fibers to enhance its user's capabilities. Ryuko escaped and retreated to the ruins of the mansion out of shame, where she found Isshin's secret laboratory. Within was the garment Isshin had made for Ryuko, a sentient "kamui" that she would later name Senketsu due to his literal bloodthirst.
Partnering with Senketsu, Ryuko spent her days at Honnouji Academy carving through the ranks of the club presidents and destroying their Goku uniforms one by one to reach Satsuki. Along the way, she met and clashed with the agents of Nudist Beach—Tsumugu Kinagase and Aikuro Mikisugi—as well as the "Elite Four," Satsuki's most trusted agents in the student council and the wearers of four of the five Three-Star Goku uniforms in existence. Satsuki donned the kamui Junketsu, created by Isshin as a prototype for Senketsu, to oppose Ryuko. When not fighting for her life at the academy, Ryuko bunked with Mako's family, and soon became an honorary member of it despite the dubious inclinations of Mako's father and brother.
Moments before Ryuko's final duel with the last member of the Elite Four, Nui Harime appeared on Ragyo's behalf to test Ryuko, bearing the other half of the Scissor Blade. Ryuko became so enraged when she learned the truth that she merged with Senketsu to become a hideous monster, and it was all Satsuki and Nui could do to hold her off. As Aikuro and Tsumugu prepared to put her down with a specially-prepared bullet, Mako rushed in and calmed Ryuko down, returning her to her normal state.
After recovering from her ordeal, Mako refused to don Senketsu again even while Honnouji Academy was gearing up to take over the final three schools that had not yet fallen under their control (and to destroy the Nudist Beach home base) until a disguised Nui propelled her into action and used the opportunity to tear Senketsu to pieces. Satsuki distributed the remains of Senketsu among her forces to power them up for the raid, and so Ryuko followed close behind her army, snatching the pieces of Senketsu and painstakingly reconstructing him until she faced off against a Satsuki powered up by the final piece. Ryuko barely managed to reclaim it and repair her partner, ultimately tying with her rival in their duel. Satsuki left to prepare Honnouji Academy for the arrival of Ragyo and the enactment of her mother's plan to feed all of humanity to the Life Fibers, leaving Ryuko in the ruins of Osaka.
Aikuro and Tsumugu informed Ryuko and Mako of the true nature of the Life Fibers and implored them to stop Ragyo. The four made it back to Honnouji just as Ragyo sprung her trap, but Satsuki betrayed her, crucifying her mother and releasing the populace of Honno Town from the Life Fiber trap. Ragyo was undaunted even by her decapitation at the hands of Satsuki and called upon the COVERS, which consumed most of Honno Town—including Mako. Ryuko, Satsuki, and the Elite Four all fought the combined forces of Nui, Ragyo, and the COVERS to no avail. During the battle, Ragyo pulled out Ryuko's heart and revealed that Ryuko was part-Life Fiber and her own daughter. Ryuko recovered her heart before falling unconscious, and Satsuki's forces retreated.
One month later, Ryuko woke up and saved the besieged Nudist Beach base from COVERS—rescuing Mako in the process—but was so filled with self-loathing that she refused to don Senketsu and abandoned her allies to return to Honnouji Academy, intent on destroying Ragyo and Nui. While her allies followed her and rescued the captured Satsuki, Ryuko was overpowered and forced into Junketsu, given false memories of happiness while Nui sewed the kamui into the fabric of Ryuko's Life Fibers, making it life-threateningly dangerous for Ryuko to take Junketsu off or escape its influence.
The remaining defenders of humanity were forced to pull back, but their reprieve did not last long as a crazed Ryuko arrived and began destroying their ship, and was soon followed by Nui. After a pitched battle with Satsuki wearing Senketsu, Mako entered Ryuko's mind through Senketsu and pleaded with Ryuko to stop. Ryuko returned to her senses, tore off Junketsu, and cut off Nui's arms, bypassing Nui's Life Fiber regeneration with the now-completed Scissor Blade having stolen the other half back from Nui.
Nui escaped with the help of Ragyo's personal assistant, Rei, and Ryuko and her allies prepared for a final assault. Ragyo appeared on the horizon, riding the source of all Life Fibers towards Honnouji Academy to transmit the Original Life Fiber's signal across the planet and activate the Life Fibers she had hidden in her clothing brand, which would've completed the Life Fibers' conquest of Earth. Satsuki and Ryuko intercepted her before she could reach Honnouji Academy, and with the help of all their allies, destroyed the Original Life Fiber.
However, Ragyo retreated to Honnouji to enact her backup plan, donning the ultimate kamui Shinra-Koketsu. Absorbing Rei to use the kamui's power, Ragyo rid her enemies of their Life Fiber-granted abilities once they arrived to finish her off. To defeat her, both Ryuko and Senketsu came to terms with their dual nature and overcame the Shinra-Koketsu's domination, cutting Rei out of the kamui and negating Shinra-Koketsu's nullification effect. Without the Shinra-Koketsu paralyzing them, Ryuko's allies destroyed the transmitter, and Ragyo was forced to sacrifice Nui to resurrect the Original Life Fiber, absorbing it to heal herself and flying to space to directly activate the satellite.
After promising to take Mako on a date once she came back, Ryuko absorbed all the remaining Life Fibers in her allies' uniforms and pursued Ragyo, where she defeated her, absorbed the Shinra-Koketsu, and used the kamui's abilities to force the Life Fibers that threatened to cover the planet to stand down. Ryuko then descended back to Earth, Senketsu sacrificing himself to cushion her descent. Ryuko's allies caught her, and weeks later, all the remaining residents of Honno Town were moving on. Rei reappeared to confront the Elite Four with their past selves, created from the fear etched in the memories of the students they terrorized. With the help of Mako and the spirit of Senketsu, the Elite Four defeated themselves and Ryuko destroyed the now-animate Honnouji Academy with the Scissor Blades, leaving the sinking island behind for new horizons with the Mankanshoku family—which included the date she promised Mako.
